For Location Production & Rental Clients: Please complete the Rental Agreement and have your insurance company issue us a certificate of insurance meeting the following requirements:

A) All Risk coverage for full replacement value in the minimum amount of ($150,000.00) that names Clark Media as loss payee and additional insured.

B) Liability coverage for the minimum amount of ($1,000,000.00)
